Why Combining Prolotherapy with Hydro-Release Gives Better Results Than Either Alone

One of the most consistent findings in our clinical experience at Proback is that combination treatment – addressing multiple components of a pain presentation simultaneously – produces significantly better and more lasting results than any single treatment used in isolation. Nowhere is this more clearly demonstrated than in the combination of Prolotherapy and Hydro-Release.

In this post we explain what each treatment does, why they work so well together and which patient presentations benefit most from this combined approach.

What Prolotherapy Does

Prolotherapy is a regenerative injection technique that uses a natural 15% dextrose solution injected precisely into damaged ligaments, tendons and soft tissue to stimulate new collagen production and structural repair. The dextrose triggers a controlled healing response – the body recognises the injection, lays down new collagen fibres and progressively strengthens the damaged structures from within.

Prolotherapy works at the structural level – repairing and strengthening the ligamentous and soft tissue framework that provides stability to a joint or spinal segment. It addresses the underlying instability and tissue damage that is often the root cause of chronic pain rather than managing the symptoms.

What Hydro-Release Does

Hydro-Release is an ultrasound guided procedure that targets fascial adhesions and scar tissue – the bound, thickened layers of connective tissue that form between muscle groups, around nerves and between fascial planes following injury, surgery, inflammation or chronic repetitive strain.

Using our SonoSite ultrasound machine we visualise the affected fascial layers in real time and inject a natural dextrose solution at the precise point of restriction – gently separating the bound layers from within and restoring the normal gliding movement of the structures passing through the affected area. The release is visible on screen as it happens.

Why Chronic Pain Involves Both Problems

The key insight behind combining these two treatments is that chronic musculoskeletal pain almost always involves both structural damage and fascial restriction – and treating only one while ignoring the other produces incomplete results.

When a joint or soft tissue structure is injured or becomes unstable two things happen simultaneously. The structural tissues – ligaments, tendons, joint capsules – become damaged and lax, losing the integrity that provides stability. And the surrounding fascial layers respond to the injury by laying down scar tissue and adhesions that progressively restrict movement, compress nerves and create a secondary pain source alongside the original structural damage.

Standard regenerative treatment addresses the structural component. But if the fascial restrictions remain they continue to compress the structures that have been repaired, restrict the movement that signals healing completion and maintain the abnormal mechanics that contributed to the original injury. The structural repair is incomplete because the environment around it has not been restored.

Conversely – releasing fascial restrictions alone without addressing the underlying structural damage that allowed them to develop means the adhesions tend to recur as the unstable joint continues to move abnormally and generate new inflammatory responses.

How the Combination Works

When Hydro-Release is combined with Prolotherapy in the same treatment session the results are consistently better than either treatment alone because each addresses what the other cannot.

Hydro-Release clears the fascial restrictions and restores normal tissue gliding – creating the mechanical environment in which the Prolotherapy repair can develop properly and reach its full potential. The new collagen stimulated by Prolotherapy is laid down into a tissue environment where normal movement mechanics have been restored rather than into a restricted, adhered environment where the same stresses that caused the original damage are still present.

Prolotherapy simultaneously addresses the underlying structural instability that is maintaining the fascial response – reducing the abnormal movement that drives ongoing fascial adhesion formation and creating the structural stability that allows the fascial release to hold.

The Role of Neuro Prolotherapy

For many patients with chronic pain a third component exists alongside the structural damage and fascial restriction – chronic nerve sensitisation. The affected nerve pathways have become hypersensitised through prolonged compression, entrapment or inflammatory exposure and are generating pain signals independently of the structural cause.

What to Expect from Combined Treatment

When Prolotherapy and Hydro-Release are combined in a treatment session the session takes slightly longer than either treatment alone – typically 30 to 45 minutes depending on the number of areas treated. Most patients find both procedures very well tolerated – the Hydro-Release in particular is often described as providing immediate relief as the restriction is released.

A mild ache or stiffness for 24 to 48 hours after each session is normal and indicates both healing responses are active. Results develop progressively across the course – most patients notice meaningful improvement from sessions two to three and continue to improve for several weeks after the final session as the new collagen matures and the fascial environment stabilises.
