What Is Prolotherapy?
Prolotherapy – short for proliferative therapy – is a regenerative injection technique that uses a natural dextrose (sugar) solution injected precisely into damaged ligaments, tendons and soft tissue to stimulate new collagen production and tissue repair. The dextrose solution triggers a localised healing response – the body recognises the injection as a mild irritant and responds by laying down new collagen fibres, tightening and strengthening the damaged structures from within.

We cannot regenerate cartilage that has been completely lost – and we would never claim otherwise. What Prolotherapy can do is strengthen the surrounding ligamentous and soft tissue structures that are under enormous stress as a result of the cartilage loss, reduce the abnormal joint mechanics that are accelerating further degeneration and create a significantly better joint environment. For many patients this translates into meaningful and lasting reduction in pain and improvement in function – even in cases that have been told hip replacement is the only remaining option.

What Does a Course of Prolotherapy for Hip Pain Involve?
A typical course of Prolotherapy for hip pain at Proback involves three to six injection sessions spaced three to five weeks apart. Each session is performed under ultrasound guidance and takes approximately 20 to 30 minutes. Before any needle is introduced we anaesthetise the skin and surrounding tissue with procaine or lidocaine – most patients find the procedure very well tolerated and are genuinely surprised by how manageable it is.
A mild ache or stiffness around the hip for 24 to 48 hours after each session is normal and indicates the healing response is active. Prolotherapy vs Steroid Injections for Hip Pain
Steroid injections are commonly offered for hip pain – both on the NHS and privately. They work by suppressing the inflammatory response in the joint – which can provide meaningful short term relief. However studies increasingly show that repeated corticosteroid injections accelerate cartilage breakdown and tendon degradation over time. The relief becomes shorter with each injection. The underlying damage continues to progress.

A double blind randomised controlled trial comparing dextrose Prolotherapy against corticosteroid injection demonstrated significantly better outcomes for dextrose at six months and beyond – with the corticosteroid group showing superior short term relief but the dextrose group producing more durable structural improvement.
