Both the franchise and franchisees operate as joint data controllers with access to your personal information. They are however restricted to accessing the information relevant to the clinic you attend. Should you wish to attend treatment at more than one clinic, individual franchisees can be permitted access to your records.

The Personal Data we process and what we do with it: In order to provide you a health care service we essentially are entering into a contract with you to deliver professional care, the provision of health-related services as a chiropractic clinic. In addition, we will only examine or treat you with your explicit consent. All children under the age of 16 years must have the consent of their parent or legal guardian before treatment is carried out. Part of this agreement includes a need for us to store and process your personal data as well as a legitimate interest in contacting you via phone, email or SMS. We don’t require you to opt in however we will always give you the opportunity to opt out of receiving communication.
We record and use the following categories of personal data: name, address, telephone numbers, email address, date of birth, health information including medical history, diagnosis and treatment data.
Third Parties: We may share anonymised personal information with only our chiropractic partners, Chirotouch (Patient management software), PostureRay (Patient scanning software), Oligoscan (Nutritional Health Scan), Biokinmetrics and TigerView (X-ray management software) who use this to analyse information to allow us accurate scanning results (PostureRay and Oligoscan), accurate record keeping and patient management (Chirotouch) and accurate x-ray guidance (Biokinmetrics and TigerView).
The type of information collected may include: Name, Address, Phone number, E-mail address, Demographic information.
ProBack Clinic Centres Ltd uses the personal information you provide to:
– Register you in our software programs,
– Create and maintain accounts,
– Answer your e-mails,

– Communicate with you with regards to your account, send surveys, clinic events, newsletter or send marketing materials.
Google Analytics: When someone visits www.proback.com we use a third-party service, Google Analytics, to collect standard internet log information and details of visitor behaviour patterns. We do this to find out things such as the number of visitors to the various parts of the site. This information is only processed in a way which does not identify anyone. We do not make, and do not allow Google to make, any attempt to find out the identities of those visiting our website.
Google Maps: These are Google Maps third party cookies, which are unique identifiers to allow traffic analysis to Google Maps.

Retaining your Personal Data: Whilst you are receiving treatment from our clinics we will continue to store and use your personal data. The retaining of data is necessary where required for contractual, legal or regulatory purposes or for our legitimate business interests and apply irrespective of the form in which records are held. We will retain your medical records at the end of any contractual agreement indefinitely. This data will be retained for the protection of our patients and practitioners by providing a history of the healthcare received.
Your Rights: As we process your personal data, you have certain rights. These are:

You may request a copy of your data at any time. Please make such a request in writing to the relevant ProBack Clinic where you attent, whose details are shown above.

We will need to verify your identity, so we may ask for a copy of your passport, driving license and/or recent utility bill. If you believe any of the personal data we hold on you is inaccurate or incomplete, please contact the relevant clinic directly and any necessary corrections to your data will be made promptly. If you believe we should erase your data, please contact the relevant clinic directly whose details are shown above.
Data Breaches: Should your personal data that we control be lost, stolen or otherwise breached, where this constitutes a high risk to your rights and freedoms, we will contact you without delay.
